IDENTIFICATION
For those who do not or cannot have a driver’s license, license, an I.D. is still necessary. This can be obtained any Florida State Driver License office. Cost is $12.00 or $25.00 and you can make an appointment or walk-in. Offices are locate at the following locations:

You MUST have one of the following:
- United States Birth Certificate (original or certified copy).
- Valid United States passport.
- Alien Registration Receipt Card (Green Card) (Form I551).
- Employment Authorization Card, issued by the U.S. Dept. of Justice, (Form 1688b).
- Proof of Non-immigration Classification, provided by the U.S. Dept. of Justice (Form I94).
A secondary identification document is also required and you can use one of the following:
1. School record stating date of birth, which must contain registrar’s signature.
2. Transcript of birth record filed with a public officer charged with the duty of recording certificates.
3. Baptism certificate which shows date of birth and place of baptism.
4. Family bible record or birth announcement in a baby book.
5. An insurance policy on the customer’s life which has been in force for at least two years and has the month, day and year of birth.
6. A military or military dependent identification card.
7. Florida or out-of-state or country driver license, valid or expired.
8. Florida license record or identification card record.
9. Selective service registration card (draft card).
10. Florida vehicle registration certificate (HSMV 83399, owner’s copy) obtained from the same dept. to which you are going, a tax collector’s office or an authorized tag agent where the customer’s vehicle was registered, Florida or out-of-state registration certificate if name and date of birth are shown.
11. Florida and out-of-state non-driver identification cards (state issued).
12. Receipt copy of your last Florida driver license issuance.
13. Immigration form I571.
14. Federal form DD214 (military record).
15. Marriage certificate.
16. Court order which includes legal name.
17. Florida voter registration card, which has been issued in the past three years.
18. personal identification by an examiner or by a person well known to an examiner.
19. A social security card.
ORANGE COUNTY SHELTERS
Coalition for the Homeless- 639 W. Central Blvd. Pavilion shelter has 500 floor spaces for men. No charge. Phone: 407-426-1250. Family shelter has 30 rooms for families with children. No charge. Phone: 407-841-2824. Women’s shelter has 40 beds. No charge. Phone: 407-422-9735. There is a 3-bed infirmary. Also a 10 bed dorm for men participating in a drug & alcohol program. Call for appointment with Johnny Little to discuss, if interested & needed in drug & alcohol program for men.
Salvation Army- 416 W. Colonial Dr. Men’s Lodge has approximately 85 beds. First 3 nights are free, then $8.00 per night. Need valid picture ID. Check in is at 4:00 p.m. and it is first come-first served. There is a 2-luggage bags limit- no paper bags, plastic bags, etc. only luggage bags. Women & children lodge has 68 beds. It is next door at 410 W. Colonial. Check in is at 4:30p.m. ID. is required but may allow 3 days to obtain. First come, first served. Those with employment get 3 free days, then pay $8.00 night for single women and $5.00 per night for mom with children. Those who find employment must pay within 7 days-on first paycheck. This is a working program with emphasis on working and saving.
Orlando Union Rescue Mission- Men’s shelter 410 W. Central Blvd. 407-423-3596. First time visit, first 5 nights free, then $6.50 per night. One (1) free night per month. Special payment arrangements made for newly employed. Vouchers available to get clothing from bargain store. Family shelter for women & children 1525 W. Washington St. 407-422-4855. To stay here residents are expected to make efforts at gaining employment and save money. There are 150 beds for women and children. Women can also receive vouchers for clothing but need to call the phone for men’s shelter to arrange.
Christian Service Center- 808 W. Central Blvd. Fresh Start shelter for men. Residential and rehabilitation program for employable men $25.. entry fee then $70.00 weekly. Call for more info. Also see food pantries and crisis services. 407-425-2523.
Women’s Residential & Counseling Center- 107 E. Hillcrest St. Shelter for battered/homeless women & children. 407-425-2502. This is a 12-14 month program. Women must have had a full-time job for at least one (1) month prior to joining. Cost for single is $40.00 weekly/mothers w/children are $50.00 weekly.
Anthony House- 6215 Holly St. Zellwood Phone: 352-383-5577. One (1) dorm for men with 10 bunks. Three (3) dorm for women and children with 8 bunks each. Cribs available. DO NOT call until released. If released call for phone screening. Bed can only be held for 3 hours after acceptance. If you are running later than 3 hrs. call to extend. Will help with employment, daycare/school for children and counseling. Gates close at 8:00 pm. There are mandatory classes for GED, NA, AA, anger management and parenting Children in residence go to Zellwood elementary.

FOOD STAMP
In order to apply for food stamps, you must go to one of the local service centers listed below. You will be asked to fill out an application and leave it there. On rare occasions, they may then give you an interview appointment on the spot. Usually you will be asked to leave your application and an interview time will be sent to you by mail. If you are not sure where you will be staying, use the address of a relative or get a post office box because without regular mail delivery, you will not know about the appointment time when you are notified.
For the interview, bring your social security card, birth certificate and your jail release papers to explain why you have not been working. You will be sent to register with job services. As an able-bodied, working adult, you may be eligible to receive food stamps for about 2-3 months. You must be actively looking for employment.

In order to apply for food stamps, you must go to one of the local service centers listed below. You will be asked to fill out an application and leave it there. On rare occasions, they may then give you an interview appointment on the spot. Usually you will be asked to leave your application and an interview time will be sent to you by mail. If you are not sure where you will be staying, use the address of a relative or get a post office box because without regular mail delivery, you will not know about the appointment time when you are notified.
For the interview, bring your social security card, birth certificate and your jail release papers to explain why you have not been working. You will be sent to register with job services. As an able-bodied, working adult, you may be eligible to receive food stamps for about 2-3 months. You must be actively looking for employment.

In order to apply for food stamps, you must go to one of the local service centers listed below. You will be asked to fill out an application and leave it there. On rare occasions, they may then give you an interview appointment on the spot. Usually you will be asked to leave your application and an interview time will be sent to you by mail. If you are not sure where you will be staying, use the address of a relative or get a post office box because without regular mail delivery, you will not know about the appointment time when you are notified.
For the interview, bring your social security card, birth certificate and your jail release papers to explain why you have not been working. You will be sent to register with job services. As an able-bodied, working adult, you may be eligible to receive food stamps for about 2-3 months. You must be actively looking for employment.

In order to apply for food stamps, you must go to one of the local service centers listed below. You will be asked to fill out an application and leave it there. On rare occasions, they may then give you an interview appointment on the spot. Usually you will be asked to leave your application and an interview time will be sent to you by mail. If you are not sure where you will be staying, use the address of a relative or get a post office box because without regular mail delivery, you will not know about the appointment time when you are notified.
For the interview, bring your social security card, birth certificate and your jail release papers to explain why you have not been working. You will be sent to register with job services. As an able-bodied, working adult, you may be eligible to receive food stamps for about 2-3 months. You must be actively looking for employment.

FOOD STAMP
In order to apply for food stamps, you must go to one of the local service centers listed below. You will be asked to fill out an application and leave it there. On rare occasions, they may then give you an interview appointment on the spot. Usually you will be asked to leave your application and an interview time will be sent to you by mail. If you are not sure where you will be staying, use the address of a relative or get a post office box because without regular mail delivery, you will not know about the appointment time when you are notified.
For the interview, bring your social security card, birth certificate and your jail release papers to explain why you have not been working. You will be sent to register with job services. As an able-bodied, working adult, you may be eligible to receive food stamps for about 2-3 months. You must be actively looking for employment.
